Bug Description

For the individual case, In Additional medical information section, phone number and email of responsible clinician is not validated.

Reproduction Steps

1. Select the case which has additional medical information(disease Acute Flaccid Paralysis).
2. Enter the Alphabets for phone number of responsible clinician
3. Enter numbers for email address. ...

Expected Behavior

Phone number should not allow to enter alphabets. Emil should not allow to enter invalid email.
