LAyout wrong when selecting "Other Symptoms" = YES

Bug Description

When selecting either "Other Clinical Symptoms" or "Other Hemmoragic Symptoms" YES a new input field appears for both of them that allows the user to specify the symptom.

It appears in the middle of other symptoms which makes it hard to see that it has appeared and makes it impossible to identifiy which of the two belongs to Clinical or Hemmoragic Symptoms as they both have the same label.

Reproduction Steps

  1. Open any case of Guinea Worm
  2. Go to "Symptoms"
  3. Select "Other Clinical Symptoms" YES
  4. Select "Bleeding or Bruising" YES
  5. Select "Other Hemmoragic Symptoms" YES

Expected Behavior

The input field should appear in a new line below the selection that made it appear and have clear labels. The input field should stand on its own (the left column should be empty) to make it clear that the form has changed.
