The changes are based on NumFOCUS requirements. I selected "custom" governance model because I was not sure whether our model is "electoral" or something else, but we can change that. I added OSGeo badge (I haven't found an official one, so I created it...) just so we have OSGeo there as well.

The links to donation are tentative, so I am marking this as draft until this resolves.
