Issue Templates: bug reports and feature requests

Just wanting to add issue templates if you are okay with it.

P.S. I love the library and appreciate the work you put into it

This looks like a great starting point for the template. 👍 I didn't merge it right away, because I want to flesh out the wiki a little bit, it's non-existent at this time. I will take this template after there's more a user can do to troubleshoot their issue.
