Update all provided example inputs/outputs

This PR fixes #10 by adding the correct input file which was overwritten, and renaming the output file accordingly. The cmdREADME.md file is also updated to reflect the correction. I also re-ran the remaining examples, so that all provided example output files were generated with the same version of ITM and ITMDrvr.
