apns feedback mistakes?

When I receive a token from apns feedback, I just remove the token from database, and no more messages will be pushed to this token. But I have observed that sometimes tokens are provided by feedback while the corresponding client apps are not uninstalled on the iOS device, they are just switched to background. Why is this happening?
