Merge Ghost 1.0 into Master and drop support for Ghost 0.11.x

The Ghost team are now asking all themes that were compatible for Ghost 0.11.x to be migrated over to Ghost 1.0. Therefore, going forward we should develop for Ghost 1.0.

  • Merging the Ghost 1.0 branch into master
  • Includes a bug fix causing prismJS syntax highlighting to misbehave
  • Bumped the version to 2.0.3-beta
