incorrect description provided when xref to nested list item uses format="default"

Describe the issue

When an xref points to an item in a nested list using the "default" format (e.g., <xref target="test1" format="default"/>), the list number in the output is incorrect. For example, the output should be "Section 2.4, Paragraph 4, Item 1.a" instead of "Section 2.4, Paragraph 4, Item 1.2.1".

Workaround: use format="counter" to get the correct item number (e.g., <t>In Item <xref target="test1" format="counter"/>...)
