Thread by subject for new messages not working

If a new message to a list is part of an existing thread, identified only by the identical base subject line, and not "In-Reply-To" or "References" headers, the new message is not recognized as being part of the existing thread.

1. To do this kind of correlation may require computing threads for the entire list, each time a new message arrives

2. What about messages with the same base subject that aren't actually the same thread? ie. announcements

As part of this issue, we should document (in the code) how threading by subject line works
