Expect: 100-continue causes body read to hang

hyper normally sends the 100 Continue response when first polling the request body. That doesn't appear to be happening. Possibly related to hyperium/hyper#3827.

Thanks for the report, do you have a reproduction (and does it also reproduce with raw hyper?).

Sorry for the wait. See #19 for a test case. There are several tests around this in Hyper, so it does work there, at least when reading the body in the traditional way. hyperium/hyper#3827 shows that reading it a different way can trigger the issue. I expect changes will be needed on both sides.
