I get teleported to a random place after connecting the portals

Describe the issue

Every once in a while when I walk through an immersive portal I get teleported to a random place nearby, even though I already connected it to another portal. Is it because I walked through too many portals before? It is very frustrating because I just want to walk through the thing I made.

Looks like the player entered swimming state. Maybe a collision issue.
