Automatically give new issues and PRs a needs-triage label

Too many issues and PRs get no attention at all. It would be good to ensure that new submissions aren't left hanging indefinitely and a label like this could help us reduce response times.

I don't know how this is usually done. Issue templates maybe? There are probably other things we could do in this general area.
