Tenant wide deployment side effects

I deployed this solution tenant wide and had no issues for weeks but recently noticed the following:

the item context menu in libraries no longer opens when the ellipsis is clicked the top toolbar in libraries doesn't appear the settings cog sometimes doesn't display pages display an error referencing require.js and a timeout loading certain modules after about 30 seconds

When the "enable" box in the app catalog is unchecked, everything works fine. I did do a second build of the extension without tenant deployment so I could inject a stylesheet on specific sites via add an app. I thought at first that might cause a conflict but if I disable that, I still see the issues. Only when the initial tenant wide app is disabled do I get back to normal.
