Clean up documentation and reading of RulesHeader

Found some cases where fields are documented as MUST be 0, but are actually 1 in the store.

This should be a good opportunity to move away from from just looking at exported RWZ files and actually extract the binary from the streams PR_RULES_TABLE property

MFCMAPI should be a good use case here or OutlookSpy
