With the possible exception of ambient mode, the iptables inside a pod (Istio) and iptables on the node (kube-proxy) have no correlation at all. I think ambient handles this fine...

Looks like the cluster init is slow for some reason. Might be good to get `trace` log dump of envoy to see what it is doing during init
Istio doesn't use active health check
it's just explicitly passed as healthy in the xds
