Indicator: Saving target periods throwing error

Current behavior Create target period is throwing 'Invalid form' error

To Reproduce Steps to reproduce the behavior:

1. Go to Indicator list
2. Click on an Indicator and go to 'Targets' tab
3. Fill in the information in the form
4. Add Target frequency and periods with targets
5. Save the form
6. See error 'Invalid form'

Expected behavior The target periods can be saved without any erro
