Fix structure to actually make use of :ok tuples

We are not taking advantage of {:ok, } and {:error,} flow that can greatly simplify a lot of the flow control in elixir. Not trivial to fix now but we can start making inroads.

Do you mean for the command flow? Would love to pair on converting one, and then we can use that as a template for other changes.
