Epic: Support all Questionnaire item types

As a form designer I can build question items of multiple types so that I could get high data quality.

Should have

• [x] boolean
• [x] decimal
• [x] integer
• [x] string (shorter texts - input)
• [x] text (longer texts - textarea)
• [x] choice (ValueSet)

May have

• [x] date
• [x] time

Nice to have

(but really not expected)

• [x] dateTime
• [x] open-choice
• [x] attachment
• [ ] reference
• [x] quantity (desimaltall med angivelse av enhet)
