Add support for FHIR packages

Is your feature request related to a problem? Please describe. Torinox only supports a local folder as the validation scope - having support for packages as well would be great.

Describe the solution you'd like

  1. Explicit option to load a package.json and add those packages to the context
  2. Automatic load of the package.json when the file is present in the local folder scope

Additional context Need @mharthoorn's package library to be released for this first.
