Request to become coordinator

We should have a form for users who wish to start a campaign as an organizer. Currently, new coordinators need to submit a request on the Commons page and are added manually by me or Leila.

Here is the information I currently need to set up the campaign:

• Campaign name
• Start and End date and time of the campaign in UTC
• User-id of coordinator(s)
• My own user-id should ideally be added to all campaign by default until a future point where my help is not needed)

I think that it would only make sense if the system can automatically send someone (me?) an e-mail every time a new request to become coordinator is submitted. Otherwise, it will become a pain to monitor these requests. Watching the page on Commons is much easier.
