Machine index migration tool

Currently Vagrant stores it's machine index in a json file ($VAGRANT_HOME/data/machine-index/index). For Vagrant-agogo we are moving to be database backed.

The difference between these two methods needs to be resolved, users must have a upgrade/downgrade path that does not loose their data.
