Prevent accidental overwrites with ld-decode

In order to prevent accidental overwrites with ld-decode the tool should check if the specified output file already exists and exit without output if it does. For the purpose of scripting support an option such as --overwrite should be included to override this default action. This will be more in-line with the tools that already provide overwrite protection.
