/etc/pam.d/sudo seemed to have a comment.

Seems to work well for me, even when added after the comment (edited with vim). I guess your issue was unrelated to the comment and had something to do with either bad syntax or something like encoding or line separators.

The error I got from console was: "cancelled by another authentication"
