/metadata endpoints with types

Overview

We should have a /metadata endpoint that contains the id and name of each facilitytype, urltype and phonenumbertype. The client can get that info up front and we won't have to include it in every api response.

Action Items

• [ ] create /metadata endpoint that returns type ids and names
• [ ] once client is using the endpoint, remove type info from the other endpoint responses
