consider pinning supervisor requirement to match ubuntu packages

Installing supervisor in the virtualenv parallels the system-level ubuntu package install. This risks being out of sync when the virtualenv might install a much more recent version.

Review the various releases of supervisor and how they relate to ubuntu-12.04 and 14.04 and pick a reasonable release range to pin our requirements.txt within.

For m5_002, limit scope to matching ubuntu 12.04.

Agreed to delay this until we bump up to ubuntu 14.04. Depends upon #238.
