Display location, callno, status, cls line and then notes when only one item

We revised the display of 'library has' message and notes so that they appear once per holding location, followed by the items. However -- especially when there is only one item -- the current display gives the impression the CLS request link is missing from the first of two items when really there is only one item. Desired display is to show: location, call number, status, and cls request on one line, then the 'library; has' and notes on the next line.
