Make the drop-down list optional

Most time input implementations (Opera, Safari, Chrome) don't show any list, so make it optional to disable.

Implement HTML5 placeholder attribute

To give a hint to the user on what to input. I.e. "hh:mm" http://dev.w3.org/html5/spec/Overview.html#the-placeholder-attribute
