Populate README API issue tracker table from public issue tracker list

Our README file has a useful table of contact points for viewing and filing issues for each API we generate. Unfortunately, many of the entries are blank, since our sources for that data are not always complete (see related issue #12722). We should use the public list of issue trackers to fill in as many of these empty entries as possible.
