[Code Health] Refactor Toolbar UIs

All the screens should have a common toolbar

  1. TermsOfService
  2. About
  3. Settings
  4. Data Sync Status
  5. Survey Selector

Parts of Toolbar

  1. Title Text
  2. Back Arrow Icon

Currently, TermOfService has no back arrow, about screen has title left aligned while the other screens title are center aligned.
