Migrate Lead Capture tool to form tool

It makes more sense to have a form type "lead form" as a template, rather than a tool.

• users report issues of agent asking for user contact when lead is disabled but form present.

Features:

• Mark field as optional. (so user can choose not to fill them)
• Ability to specify when the agent should start collection user informations (eg, at the start, middle or end of conversation)
• Add a consent phase to the form. (ask user 'do u want to share your informations' if yes fill form, if no, discard for tool)
