Toolbelt duplicated on death and with contents inside.

ToolBelt mod is a part of one of my modpacks in development, I've recently noticed that it's duplicating upon death. You die, belt stays in it's rightful spot, and another gets created and placed on the floor along with the stuff you've lost. Only thing that remain is the toolbelt in your inventory after the death happens. It should just drop, but some how the slot is not allowing that.
