Drop Media from `messages` in SDKs and document behavior
Images and other media in the messages attributes should be dropped by the SDK to prevent large media from breaking instrumentation. The fact that we will drop these should be documented in our docs. We should track the fact that we dropped this media via client reports / outcomes to a) make sure users are aware of it and b) that we know how often this happens.

@RulaKhaled now media content in MCP messages is now automatically filtered out and tracked via client reports
@naan760 we are not looking to add this for mcp, this should be added for our ai integrations, let me know if you're interested in tackling this, otherwise feel free to close the PR.
