the changes in agent mode cannot be applied to the files

I have issue when I use cursor agent mode to modify my code. The changes from agent cannot be applied to my file automatically and there are no messages telling me why, also, it even cannot be applied when i reapply it manually (by clicking the button in image). The details are in the image following.

and it also has trouble with finding the correct path of my file. is this a bug or what?
