Opus sometimes hallucinates a user response after it finishes its own response

Sometimes Opus will continue after finishing a response, with "User: " followed by hallucinated responses that Opus expects a human to respond with.

Not sure but maybe Opus could benefit from an extra stop token for "User: "?

(either way, only providing this if it would help, it's not a severe bug and I love the product!)
