Neglecting Offhand (Infinite Loop)

Steps to Reproduce (as best as you can)

Start any complicated task like gamer or stacked, bot will eventually place an item in its offhand.

Expected Behavior

Bot understands item in offhand is part of its inventory and can manipulate it to use in crafting, mining, and eating.

Actual Behavior (What lead to the bot getting permanently stuck)

Bot seems to sometimes place an item in its offhand, but this causes the bot to forget the items actually exists in its inventory. For example, the bot needs to buckets to get to the nether, but it may place the buckets in offhand. The bot forgets about the buckets it already has and continues to find and smelt iron.
