Add insert and preserve links example

As promised in this issue, here is a standalone example of inserting one PDF into another, preserving the links from the first PDF. Specifically, this shows how to update the destinations of links that point to a specific page (as opposed to something else like a heading) after insert to account for the length of the inserted PDF.
