Not applied if cart is skipped

Hello,

I have found that on some orders, the free item is missing from a qualified order. What I have discovered is that some customers are skipping the cart and moving directly on to checkout. If the cart page is not loaded, the item is not added. How can I trigger the check against the cart and add the free item if the cart is skipped?
