Add multilevel Outline support

I need to create a tool which is able to merge multiple PDFs together and provide a multilevel Outline to them. It would be very cool to have such feature!

You mean an Outline with sub-items? Possible, yeah, although I don't know how.

Yeah, it can be complicated... Tomorrow I have 5-6 hours: I will try to implement this in the library myself and make a pull request. Very likely I will not succeed in so little time, and there is another problem: this might introduce breaking changes, because the method "add_bookmark()" would not be general enough without at least an extra argument "parent" or similar :( We will see
