Unexplained 500 errors

The above log lines are odd because they are 500 status but I can't find an associated error message or stack trace.

I created a page for just a few minutes to test a new form sending responses to our test Redmine instance (running through tests before doing an upgrade to a new major version). I deleted the page after I submitted a response, so those errors are probably happening because that page only existed for a few minutes.

I tried using the "test' page that already existed, but it gave me an internal server error so I moved to a new one. I'm not sure the underlying cause for that one (/test)
