Got All Documents view working, some css changes

I'm pretty new with backbone, so you might want to check how I've implemented things, but I think I've got the 'All Documents' view working properly - there is no link to view the whole document or anything, just showing the id and entity type.

Also added some styling to the index definition list items to make it look a bit more 'ravenish'.
