form-->field-->add-->update

Added new row but not show in the form, any way to fix?

After adding a new raw on form tool I got this error any who can help me solve this : Class: FormTools\Submissions File: /home/hiremoeg/public_html/crm/global/code/Submissions.class.php Line: 1088 Error: SQLSTATE[42S22]: Column not found: 1054 Unknown column 'col_22' in 'field list'
