Unable to remove a Forem that is offline

Describe the bug

If a forem instance "goes away" and is already in the sidebar, the icon and links break, and there is no way in the UI to remove it.

To Reproduce

Tricky to reproduce unless you already have a forem instance added that goes offline. In my case, I have community.devocate.com added, but the DNS no longer resolves.

Expected behavior

Either a right-click option on a sidebar icon to remove it, or some access to the underlying settings in the UI.
