[Feature Request] Make Floating Bookmarks Default

In reference to two issues:

Anchor bookmarks to paragraph rather than page

Missing documentation for “floating” bookmarks?

Once books are reflowed, bookmarks no longer point to their original location. That's now what we would want in most applications. In fact, I'm unable to see an applications for that.

It would also help to be able to bulk convert regular to floating bookmarks.
