Fork all debugging docs out of CONTRIBUTING.md into their own doc

I also remove the table of contents, as GitHub renders a table of contents (an "outline") for all Markdown files.

The only new content is the "Print-debugging" section. The rest of the text is more-or-less intact; I just wrap some lines and fix some links.
