Adding Software (.deb file) to a team creates a duplicate or ghost file

💥  Actual behavior

Duplicate .deb installers visible in UI when uploading files. File name and version details are the same, but if you look at the API they are different. Could be a ghost file from installers that exist on other teams or were already deleted.

🧑‍💻  Steps to reproduce

1. Upload a .deb installer to a Team (You may need to upload the same or similar .deb installer to another team to reproduce the issue)
2. Note the two instances of the same file in the UI
