allow measurable ratings to be made from the measurable screen as well as the application screen

I frequently get users looking lost because they log on the measurable screen and then can't find where to link them to applications. ideally there should be a rating tab like there is on the application screens

Actually, think this may be a dupe of #3746 (which unfortunately keeps getting bumped off the priority list)

I think #3746 is slightly different as that is a bulk upload. the idea here is that the UI will allow ratings to be selected individually

two different approaches to the same issue. i am thinking of reversing the widget from the applications "ratings" section
