Remove fiaas-deploy-daemon-bootstrap entrypoint

Follow up from #163; since deploying fiaas-deploy-daemon with skipper is deprecated, the fiaas-deploy-daemon-bootstrap entrypoint and associated e2e test could be removed. Since removing this means it would no longer be possible to deploy new fiaas-deploy-daemon releases with skipper (using the workaround documented here) at all, maybe it would be a good idea to wait some predefined time (1/3/6 month(s)? I'm not sure what is an appropriate grace period) before removing it completely.

Hello @oyvindio, sorry for the late response.

From our side, we will work on moving out of Skipper next year. We want to be releasing with the sem-ver releases in April. Does it sound reasonable to you?
