preprocess .ld files

Our .ld files are almost identical except for the memory areas.

It should be possible to preprocess .ld files (and e.g. use #ifdef blocks) : https://stackoverflow.com/questions/28837199/can-i-use-preprocessor-directives-in-ld-file
