Add Navigation links and back links

Added Table of Contents and back links to all menu items improving general navigation. e.g.

I love this idea, but when going to your repo and clicking several links, it, unfortunately, doesn't seem to work...

Yeah, I figured the exact same thing but didn't have the time to go back to fix it. I'll fix it soon.
