Headings missing and links broken in documentation

Installing Kotlin locally links to sections "Windows", "macOS", and "Linux", but those links don't work, the headings of those sections are missing.

Similarly, Testing on the Kotlin track links to sections "Windows", "macOS", and "Linux", but those links don't work, the headings of those sections are missing.

PR #544 fixes that by avoiding level-1 headings and adding the prefix "h-" to the URLs of the links.
